Employee Payroll Deduction Instruction Form Use this form if your employer offers payroll deduction and you want to have money automatically deducted from your paycheck and deposited into an account in New York's 529 College Savings Program Direct Plan, or if you want to change your existing deduction. Account Information Change Form Use this form to change your name, address, phone number, or successor account owner information or to add an interested party to your account in New York's 529 College Savings Program Direct Plan. (Do this online.)
Beneficiary Change Form Use this form to change the beneficiary on an existing account or to transfer a portion of the account assets to a new beneficiary's account.
Change of Ownership Form Use this form to transfer your entire account balance to a new account owner.
Exchange / Future Contribution (Allocation) Form Use this form to move money within a Program account to a different investment option (allowed twice per calendar year) or to change investment options for future contributions (allowed anytime). (Do this online.)
Automatic Investment Plan (AIP) / Electronic Bank Transfer (EBT) Form Use this form to start, change, or stop automatic bank transfer contributions or to add or change bank information for electronic contributions to an account in New York's 529 College Savings Program Direct Plan. (Do this online.)
Additional Purchase Forms Use this form to make additional contributions to an account in New York's 529 College Savings Program Direct Plan. (Do this online.)
Incoming Rollover / Transfer Form Use this form to roll over funds from another qualified 529 plan to an account in New York's 529 College Savings Program Direct Plan.
Form W-9 Request for Taxpayer Identification Number and Certification Use this form to certify your taxpayer identification number.
Withdrawal Request Form Use this form to request a withdrawal from your account in New York's 529 College Savings Program Direct Plan. (Do this online - Qualified only.)
